Introducing Our Virtual dance program!

While we believe that in-person dance training is best, we know that many of you are not fully comfortable returning to the studio just yet. That is why we have decided to develop a fully online dance program for your dancer. Your dancer will have a unique and fun experience via Zoom for 30 minute classes. Classes will run each week on Wednesdays and several classes are available. The dancers will be able to see and say hello to their DAnce friends that they surely miss seeing every week or even make some new friends. They will be learning many skills, steps and technique as well as routines without actually performing in June. However, for those DAncers that complete the program through the last Sunday in June, they will be awarded a beautiful medal and a certificate with their accomplishment of completing the season virtually that will be mailed to you. It’s truly something to work towards!! Virtual dancers will also get registration priority over non-students for the 2022 season.

For a great class experience:
*Download the ZOOM app (it’s free to download and for you to use) Create an account and familiarize yourself with the app
*Create or clear a space for your dancer to take class. We will work with any size space, but the more room - the better!
*Enter your dancer’s name in the space provided for the class. We will only let dancers in that we recognize the name of
*Once you register, you will get class info sent to you

Costs of the program:
All costs and payments are non-refundable for any reason. All lessons must be paid for, even if missed. All lessons are due prior to the class. Payments can be sent by check or through Venmo.

Registration (for all virtual dancers) -$25 each dancer

Once registration is paid, you can use any credits that you may have with us towards your virtual lessons! Credits expire after the 2020-2021 season.

Class fee per 30 minutes $7 per dancer or take all 4 classes for $25!

  • All policies, waiver, acceptance of risk and liability extends to our virtual program and any online programs or classes we offer over ZOOM or other sites. Our VIRTUAL PROGRAM dancers are not required to purchase costumes, tights, shoes etc. Virtual Program fees include registration and class fees. All payments are non-refundable for any reason. Virtual Program registration is $25 for each dancer, even if previously enrolled at the studio. Any credits you may have can be used for Virtual Program lessons once registration is paid. All credits expire after the 2020-2021 season.

Financial Policies

    • All Lessons must be paid for at the time of the lesson or at the beginning of each month. We suggest setting up the studio as an automatic payment through your online banking. You can also mail a check to the studio or pay via Venmo. Classes must be paid for, even if missed. We do not offer make up classes. Dancers cannot participate in class if payments are not up to date. All fundraisers with full payment must be turned in by the due date as well as costume fees and deposits.
    • A $10 late fee will be automatically added to your account if fees are not paid on time (by the 10th of the following month) This includes all payment due dates.
    • All Payments, deposits, fundraisers and registrations are non-transferrable and non-refundable.
    • Any returned checks will have a fee of $25. If this makes your payment late, you will also have a late fee.
    • We will be terminating our Perfect Attendance award program
    • All dancers must pay a $5 costume deposit for each routine (including Finale) that their dancer will be in. This is non-refundable and the credit will be on your final costume bill
    • All competitive dancers (All Star Team) are required to pay for all registered routines including all classes, Solos, Duos, Trios and Privates for the entire season. Should you leave for any reason, payment will be due through Recital for all lessons, competition fees, gear, costumes and any other costs. NO EXCEPTIONS
 

Annual Recital Policies

    • All dancers are required to participate in our Annual Rehearsal and Recital. We also have a Picture Day (optional) You will be given a special packet of information before these events. You will be required to read and adhere to all grooming guidelines and other instructions within the packet.
    • Our studio has 2 shows. Based on enrollment, we will determine which routines are in which show. Your dancer might be in 2 shows. The shows would be on the same day and we will let you know by posting on our website, email and the info board which classes are in which shows. Each family will be required to purchase 5 tickets for the show (your choice of show if your dancers are in multiple shows) We require a minimum of 5 tickets per family to help pay for renting the school auditorium and classrooms for 2 days, janitors, insurances, firemen, policemen etc.
    • Female dancers will be required to purchase at least 2 pairs of tights from us for events ($15 - $20)
    • Costumes are required for all dancers. All dancers will be in our Finale and it typically costs $42-$50. All other costumes for Recital are $55-$80 each and Competitive costumes are $65-$115 each
 

Dress Code

    • All female dancers should wear a bodysuit and footless or stirrup tights. If your dancer’s tights have feet, please cut the tip of the tights and pull off of the foot and place around dancer’s ankles. This is for the safety of your dancer.
    • All Male dancers should wear gym type shorts and a shirt
    • All hair should be tied back and out of the dancer’s face.
    • Some classes require shoes. Please do not buy any shoes without speaking to someone from our staff. We recommend writing your dancer’s name somewhere in the inside of the shoe in case they are left behind.
    • Tap – Any Black Tap Shoes (All Star dancers see Katie)
    • Ballet – Ordered through us at the desk (Usually $20-$30)
    • Hip Hop – Black and White combination Sneaker. Sneakers MUST not be worn outside. Dancers do not need sneakers right away but will need them for picture day, rehearsal and recital. (All Star Dancers see Kelly)
    • No oils or colored gels in your dancers hair. Also make sure your dancer’s hair is not wet. All of these conditions can make the floor and Acro mats slippery and cause a hazard.
 

Chaperone Policy

    • All PreDancers should have someone waiting for them outside of class. A member of our staff will escort them to the bathroom and assist them (If you prefer, you can sit on the snack side and bring them yourself if they come out). At the end of class, please be ready to look for your dancer when they come out.
    • All dancers in our K-3 class that need help using the bathroom (including walking in the hallway and making it back to class) will need someone to stay (on the snack side) to help them in case they come out of class. They will also exit the class on the office side after each class. If they will be confused or need assistance to get to the next class, please make sure someone is there for them.
    • Please be on time to pick up your dancer. Please call the studio if you will be late picking up your dancer 401-727-1636. You will be charged a fee if you are late without letting us know or if we must remain open waiting for you to pick your dancer up.
    • Make sure your dancer knows to stay upstairs and wait for you. We are not responsible for dancers who are left unattended and do not follow our policy to stay in the waiting room upstairs.
    • No matter the age, we are not responsible for the dancers while they are in-between classes, before class or after class. Please make sure they know the student rules posted on the info board and in the snack area. There is no hanging out on the stairs, in the entry way or in the area where you enter upstairs. Any dancers who are disrespectful or do not follow the student rules will be required to have a chaperone with them while not in class.
 

Safety

    • Please note that our studio is under constant video surveillance for everyone’s safety
    • Please make your dancer and yourself aware of all fire exits and extinguisher locations.
    • Please be on time to pick up your dancer
    • Please be mindful of the traffic in front of the studio
    • Please do not allow your dancer to play with our alarm system keyboard or fire alarms/fire extinguishers. You will take full responsibility for any costs or charges that occur
 

General Policies

    • No pets
    • No swearing and please keep all conversations family friendly
    • Stay informed! Please check the info board, website & Facebook pages for the latest news.
    • We reserve the right to change all policies at any time including fees, dates etc
    • No water or snacks allowed in the dance rooms
    • There is a lost and found bin located in the coat room. Please check it frequently. We will donate all unclaimed items
    • If you have an older dancer that does homework or eats between classes or a dancer that stays without a chaperone, please read the rules in the “Snack Area” with your dancer.
    • ALL POLICIES WILL BE MODIFIED ACCORDING TO CURRENT SITUATIONS. FOR EXAMPLE, CORONAVIRUS GUIDELINES WILL BE RELEASED SEPARATELY AND MAY INCLUDE ALL OF OUR POLICIES PLUS MORE RESTRICTIONS ETC

    BY REGISTERING YOUR DANCER(S) OR ALLOWING YOUR DANCER TO CONTINUE DANCING AT DANCE ATTITUDES, YOU AND YOUR DANCER(S) ARE AGREEING TO FOLLOW ALL OF OUR POLICIES AND RULES. INCLUDING, BUT NOT LIMITED TO REHEARSAL, RECITAL, PICTURE DAY, COSTUMING, PAYMENTS, FUNDRAISERS, LATE FEES, SHOES DRESS CODES ETC. A copy of these policies can be found on the info board at the studio. We reserve the right to change any policy at any time. We will post any and all policy updates on the information board at the studio.

 

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

  • DANCE ATTITUDES

    I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

    I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.

    I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event.

    In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

    • I HEREBY WAIVE, RELEASE, AND DISCHARGE DANCE ATTITUDES and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography or any activity I may participate.
    • I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: DANCE ATTITUDES and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;
    • I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.

    The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

    Any activity comes with risks. Accidents can happen anywhere, at any time. We are not responsible for any injuries to any person on or off of the premises (including, but not limited to, all of Dance Attitudes, sidewalks, parking lot, common areas of the building etc) You are entering and participating at your own risk.


    I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

    PARENT / GUARDIAN WAIVER FOR MINORS (Only if student is under 18 years old) The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s participation in the activity or event, and has agreed individually and on behalf of the child or ward, to the terms of the accident waiver and release of liability set forth above. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim, or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.

    I, the undersigned, represent, acknowledge and accept that participation in the activities for which the Participant(s) is registered includes strenuous physical activity and involves foreseeable risks of serious injury (including paralysis and even death) with or without the negligence of others. We suggest all families carry their own insurance policies to cover their dancer. Participant(s), on his or her behalf, and the parent/guardians, on behalf of Minor Participant(s) as well as on their own behalf, unequivocally agree to incur and assume such risks as a condition to participation in the activities for which Participant(s) is registered. I hereby waive all claims (past, present or future), release and discharge, covenant not to sue, and agree to indemnify and hold harmless Dance Attitudes (as well as its officers, employees and agents) and the hosting site on whose premises the classes or event will be held (hereinafter collectively “Releasees”) from any and all liability, loss, cost, expense, claims, demands, actions, judgments and executions which the undersigned ever had, now has, or which the undersigned may have in the future, for personal injuries, known or unknown, and damage to property (real or personal) in any way caused by, related to, or arising out of, directly or indirectly, the activity for which Participant(s) is registered or in which Participant(s) is permitted to engage. In the event of illness or injury, I authorize Dance Attitudes (Including Kelly Garrity and Katie Mazzeo) to obtain necessary medical treatment for the Participant(s) and hereby, in my own behalf and on behalf of the Participant(s), release and hold harmless all Releasees. The undersigned represents and agrees that this waiver and release is binding not only on the undersigned, but also upon their respective heirs, representatives, administrators, executors, and assigns. This release covers every possible injury or accident of every sort and nature, whether related to the permitted activity, the equipment, the condition of the premises, or otherwise, and regardless of whether due in whole or in part to the negligence of a releasee or other Participant(s). The undersigned represent that the Participant(s) is healthy and has no physical or mental condition that would impair Participant’s ability to fully and safely participate in dance classes and events. Dance Attitudes is not responsible for participants' supervision while not in class or while changing classes. I understand that Dance Attitudes often produces promotional and other materials relating to its programs. I understand that as a participant(s) in and/or a spectator at an event the Participant(s) may be included in DVDs, podcasts, videocasts and/or photographs taken during class or event. Therefore, without reservation or limitations, I, in my own behalf and on behalf of the Minor Participant(s), hereby assign, transfer and grant to Dance Attitudes, its successors, assignees, licensees, sponsors, any television networks, and all other commercial exhibitors the exclusive right to photograph and/or videotape the Minor Participant(s) and to utilize such videotapes and photographs and Participant’s name, face, likeness, voice and appearance in advertising and promoting the studio or in advertising and promoting future classes and for sale for any and all purposes. I further understand that neither Dance Attitudes nor any third party is under and obligation to exercise any of the foregoing rights, licenses and privileges. I represent that any medication to which Participant(s) is allergic or is currently taking are listed below
